Although less than a centimetre in diameter, the ampulla region is the area of the small bowel with the highest incidence of neoplastic transformation and malignancy. Endoscopic ultrasonography is today the best tool to examine the papilla and to stage periampullary tumours. Radial and linear endosonography as well as intraductal ultrasonography have all been proven useful in locoregional staging compared to duodenoscopy, ultrasonography, tomodensitometry and magnetic resonance imaging. Fine needle aspiration can furthermore increase the accuracy for the diagnosis of malignancy, and of lymph node or distant metastasis. There are however limitations in its use, for example differentiation of benign from malignant lesions. Anatomical variations or pitfalls such as periampullary diverticulum, benign and inflammatory lesions, preceding sphincterotomy or stent placement may complicate the endosonography interpretation of the ampullary region. This review will also cover less frequent papillary anomalies and use of EUS in sphincter of Oddi dysfunction.
