Que faire face à une thrombopénie maternelle pendant la grossesse ?

La thrombopénie maternelle (<150.000 plaquettes/µL) est présente dans 5 à 10% des grossesses. Le diagnostic différentiel peut être ardu mais il est indispensable afin de pouvoir offrir un suivi prénatal et une prise en charge adaptée. En effet, la thrombopénie pendant la grossesse peut mener à des complications maternelles, fœtales et néonatales. Après avoir exclu une pseudo-thrombopénie, on différencie les thrombopénies isolées (thrombopénie gestationnelle et purpura thrombopénique immun) des thrombopénies associées à une anémie hémolytique (prééclampsie, HELLP syndrome et microangiopathies). Dans cet article, nous détaillerons ces différentes pathologies et offrirons des clés diagnostiques pour ensuite exposer les connaissances actuelles concernant le traitement, le suivi prénatal, l’accouchement, la période néonatale et le post-partum. Un algorithme d’aide au diagnostic différentiel des thrombopénies chez la femme enceinte est proposé à la fin de cet article.

(eng) [What to do in the event of maternal thrombocytopenia during pregnancy?] Maternal thrombocytopenia (<150,000 platelets/µL) occurs in 5 to 10% of pregnancies. Differential diagnosis can be difficult, but it is essential to offer appropriate antenatal monitoring and treatment. Indeed, thrombocytopenia during pregnancy can lead to maternal, fetal, and neonatal complications. After ruling out pseudothrombocytopenia, a distinction is made between isolated thrombocytopenia (gestational thrombocytopenia and immune thrombocytopenic purpura) and thrombocytopenia associated with hemolytic anemia (pre-eclampsia, HELLP syndrome and microangiopathies). In this article, we will describe these different pathologies in detail and offer diagnostic keys, before outlining current knowledge regarding treatment, antenatal monitoring, delivery, as well as neonatal period and post-partum periods. An algorithm to facilitate the differential diagnosis of thrombocytopenia in pregnant women is proposed at the end of this article.
