Hair testing reveals that cocaine is frequently adulterated with levamisole Blood determination of levamisole remains difficult, with a short half-life of 5.6 h The possible role of levamisole in inducing MIL or more particular brain lesions (Susac or Susac-like syndromes) remains speculative but further epidemiological investigations are required Atypical neurological presentation after cocaine use should be investigated by brain MRI Shorter duration of exposure and lower doses for MIL in comparison with vasculitis or agranulocytosis? Immunologic mechanism? Immunotherapy (steroids, immunoglobulin, plasmapheresis) may be beneficial in documented cases of MIL
