A case of subperiosteal osteoid osteoma of the tibia misdiagnosed as stress fracture based on a history of multiple trauma and the results at MRI and bone scintigraphy is described. The nidus was well demonstrated at CT and the diagnosis was confirmed at histology. The role of different imaging techniques and their pitfalls are discussed.
