Recent reports showing the long-term benefit of the pulmonary autograft over conventional aortic valve (AV) replacement with a prosthesis in adults have renewed the interest in the Ross operation, which is regaining momentum. Nonetheless, above the technical complexity of the reimplantation of the pulmonary autograft in aortic position with or without an external support, one of the main concerns against the Ross is the risk of failure in the form mainly of root dilatation, valve regurgitation, or both, over time.
