OBJECTIVE: To evaluate the ovarian response to stimulation for IVF in endometriosis patients who have previously undergone laparoscopic treatment of peritoneal and/or ovarian endometriosis (CO2 laser vaporization). PATIENTS AND METHODS: Retrospective study of 455 patients undergoing IVF. The study group, made up of 127 endometriosis patients, was divided into 2 subgroups: Ia: 42 women with peritoneal endometriosis, treated by laparoscopy, who underwent 71 IVF cycles; Ib: 85 women with ovarian endometriomas, treated by vaporization of the internal wall, who underwent 187 IVF cycles. The control group, consisting of 328 women, was also divided into 2 subgroups: IIa: 193 women suffering from tubal infertility who underwent 422 IVF cycles; IIb: 135 women with idiopathic infertility who underwent 275 IVF cycles. RESULTS: The ovarian stimulation parameters (number of gonadotrophin ampoules, number of follicles and mature oocytes, maximum estradiol concentrations) were not significantly different in the various subgroups. The number of embryos obtained and transferred per cycle, the fertilization rates (group Ia: 61.81%; Ib: 60.90%; IIa: 62.48%; IIb: 57.99%), the implantation rates (group Ia: 17.72%; Ib: 15%; IIa: 13.94%; IIb: 18.05%) and the clinical pregnancy rates (group Ia: 32.39%; Ib: 37.40%; IIa: 27.49%; IIb: 30.18%) were not statistically different in the studied subgroups. DISCUSSION AND CONCLUSION: The theoretical risk of loss of ovarian cortex when treating endometriotic cysts can be eliminated by the technique of vaporization of the internal wall of the endometrioma.IVF outcomes are similar in patients treated for endometriosis and those presenting with unexplained or tubal infertility.

Objectif. – Évaluer la réponse ovarienne à une stimulation pour FIV chez des patientes endométriosiques ayant bénéficié au préalable d’un traitement laparoscopique d’endométriose péritonéale et/ou ovarienne (vaporisation au laser CO2).
Patientes et méthodes. – Étude rétrospective de 455 patientes ayant bénéficié d’une tentative de FIV. Le groupe d’étude composé de 127 patientes endométriosiques a été divisé en deux sous-groupes : Ia : 42 femmes ayant de l’endométriose péritonéale traitée par laparoscopie et ayant bénéficié de 71 cycles de FIV ; Ib : 85 femmes présentant des endométriomes ovariens traités par vaporisation de la paroi interne ayant bénéficié de 187 cycles de FIV. Le groupe contrôle composé de 328 femmes est divisé en deux sous-groupes : IIa : 193 femmes ayant une stérilité d’origine tubaire ayant bénéficié de 422 cycles de FIV ; IIb : 135 femmes ayant une stérilité idiopathique ayant bénéficié de 275 cycles de FIV.
Résultats. – Les paramètres de stimulation ovarienne (nombre d’ampoules de gonadotrophines, nombre de follicules et d’ovocytes matures, concentration maximale d’œstradiol) n’étaient pas significativement différents dans les sous-groupes.
Le nombre d’embryons obtenus par cycle et transférés, les taux de fertilisation (groupe Ia : 61,81 % ; Ib : 60,90 % ; IIa : 62,48 % ; IIb : 57,99 %), les taux d’implantation (groupe Ia : 17,72 % ; Ib : 15 % ; IIa : 13,94 % ; IIb : 18,05 %), et le taux de grossesse clinique (groupe Ia : 32,39 % ; Ib : 37,40 % ; IIa : 27,49 % ; IIb : 30,18 %) n’étaient pas statistiquement différents dans les sous-groupes étudiés.
Discussion et conclusion. – Les risques théoriques de perte de cortex ovarien lors du traitement des kystes endométriosiques peuvent être éliminés par la technique de vaporisation de la paroi interne de l’endométriome.
Les résultats en FIV sont similaires chez les patientes traitées pour endométriose et les patientes présentant des stérilités tubaires ou inexpliquées.
