The authors report the evolution of a femoral neck pseudarthrosis secondary to a coxa vara in a child with renal osteodystrophy due to polycystic dysplasia. After kidney transplantation, despite normalisation of the kidney function, the coxa vara progressed further on the left side where it was complicated by spontaneous fracture of the femoral neck which failed to heal. Proximal femoral valgus osteotomy, as described by Pauwels, healed the nonunion and achieved a general improvement of the axis of the left lower limb.
